This Membership Agreement ("Agreement") is between you ("Member") and the Company. By signing up you agree to the terms below as well as our Terms of Service and Privacy Policy, which are incorporated here by reference.
Your membership gives you access to the services and benefits described in your selected plan, including the Annual Home Health Assessment, account credits, priority scheduling, and use of the member portal. Full plan details are shown at signup and may be updated with reasonable notice.
Your membership fee is billed automatically each billing cycle to the payment method on file. See our Terms of Service (Section 10) for full billing, failed payment, and cancellation terms.
Your membership continues until cancelled. You may cancel at any time; cancellation takes effect at the end of the current billing period. See Terms of Service Section 11 for full cancellation and refund terms.
By activating your membership you confirm that you have read and agree to this Agreement, our Terms of Service, and our Privacy Policy. This acceptance is recorded with the date, time, and version number shown below.
